I-15 11400 South Exit Instructions:
Take the 11400 So. exit off I-15 and head East. Turn left onto State Street. Proceed North to 11075 So on the right hand side of the road. Enter the business complex and take the first right and then the next left. Building 16 is on your immediate righthand side. The sign on the outside of the building says South Valley Clinical Services . We are in that clinic on the East side, 2nd floor.
